 -> shallow HLS verification
 -> priority deep segment verification
 -> health memory / 3-strike failure handling
 -> clean JP.m3u

V2.4.1:
- Upstream First retained
- Clean Engine restored
- fresh upstream routes tested before old cache
- new unverified channels do not enter JP.m3u
- healthy unchecked channels are carried forward
- temporary failures keep last-known-good route
- 3 consecutive checked failures before removal
- new channels rotate through future refresh rounds
- up to 100 clean output channels
- no login/DRM/token/paywall bypass

Principle:
Upstream finds. Clean Engine decides what gets published.
